Also, more than once I've filled out the form only to be told essentially, "you've already requested one." BUT- then within a couple weeks I received it anyways! So just keep that in mind!
A couple weeks ago I got one- only it was just the outside cover! All the insides were torn out. I found out this happened to several individuals and that it was a production error. You can email them here and request another booklet.
